Intracranial venous thrombosis complicating oral contraception: treatment by anticoagulant drugs.

چکیده

The first suggestion that oral contraception might be responsible for cerebral vascular disease was made by Lorentz (1962), when he described the case of a young woman who had been taking the contraceptive pill norethynodrel (Enavid) and had developed a left parietal vascular lesion. Other authors subsequently reported similar cases. Walsh et al. (1965) were the first to prove that the underlying disorder in these cases might be venous rather than arterial thrombosis. Among their 17 cases of cerebral vascular accidents in young women taking the contraceptive pill they reported one fatal case in which there was both arterial and venous thrombosis and one fatal case of pure venous thrombosis. Atkinson et al. (1970) described five further fatal cases. of cerebral venous thrombosis, and Buchanan and Brazinsky (1970) another. Pathological studies of two more fatal cases have been reported by Poltera (1972).

Spontaneous Intracranial Hypotension Plus Cerebral Venous Thrombosis: A Case Report Study

This is a case study of a 34-year-old woman who was admitted to hospital with a history of severe orthostatic headache. She was diagnosed as having spontaneous intracranial hypotension (SIH) by undetectable cerebrospinal fluid (CSF) pressure at lumbar puncture, and with evidence of diffuse dural enhancement of the brain detected by magnetic resonance imaging (MRI). However, the contrast-enhance...

Subarachnoid hemorrhage as the initial presentation of dural sinus thrombosis.

Cerebral venous thrombosis (CVT) can be difficult to diagnose because of its wide spectrum of clinical manifestations. Its diagnosis may be further complicated when patients initially present with acute subarachnoid hemorrhage (SAH). We report on four patients with SAH revealing a CVT and discuss the role of imaging for diagnostic and pretherapeutic workup. In three women and one man presenting...

Patients with antiphospholipid antibodies and venous thrombosis should receive long term anticoagulant treatment.

OBJECTIVE To determine whether the finding of antiphospholipid antibodies in patients with venous thromboembolic episodes should influence the duration of treatment with anticoagulant drugs by mouth. METHODS A retrospective study was carried out in 19 patients with antiphospholipid antibodies and a history of venous thromboembolic episodes. The median follow up from the first venous thromboem...

Progestin-only contraception and venous thromboembolism.

Combined oral contraceptives (COC) are the most popular contraceptive method in developed countries. Since their introduction there have been numerous changes and modifications in its composition with the aim to improve safety and tolerability while maintaining contraceptive efficacy. Most of the changes have been conducted on the progestin component, since most of the combinations include ethi...

Evidence-basis for anticoagulants for cerebral sinus venous thrombosis?

Evidence-Basis for Anticoagulants for Cerebral Sinus Venous Thrombosis? To the Editor: The recent case of cerebral sinus venous thrombosis (CSVT) in the US Secretary of State Hillary Clinton brought my attention to articles that lay out the evidence-basis for the use of full dose heparin and oral anticoagulant drugs for CSVT. Unfortunately, articles published in Stroke by investigators from the...
